A resident of Baton Rouge, he died at 2:30 a.m. Sunday, Dec. 14, 2008, at his home. He was 46 and a native of Independence. Visiting at Resthaven Funeral Home chapel, 11817 Jefferson Highway, on Thursday, Dec. 18, from 5 p.m. until memorial service at 7 p.m., conducted by the Rev. Merlin Liverman. Survived by his mother, Hazel E. Levatino; father, Phillip J. Levatino Jr. and wife Helen; maternal grandmother, Annie Mae Fussell; two sons, Jason Levatino and Jeremy Levatino; and sister, Robin L. Burke and husband Patrick.
